Question: Creating help pages

It's a little confusing trying to create a help page. 

Using the tools Save as help page option is confusing and not explained very well in the help.  The table of Contents entry does not file it under a folder unless it's followed by a forward slash(/)

As an example if I wanted a help page called HELP in a directory called HELP in the table of contents I had to enter HELP/HELP in the table of contents field, it wasn't very clear but that was the only way I could get it to work.  Can someone shed some more light on creating help pages?  Maybe put the worksheet into the application center on the maplesoft website?  And place it here for other people as well. 

Can anyone point to a good link to create help pages?  or write up a little tutorial to help everyone out?
